Surrogate Inquiry Form

Although many women want to become a Surrogate Mother, we can only work with women who live in surrogate friendly states that clearly acknowledge the validity of and uphold the intent of the parties to surrogacy contracts. If you are unsure whether you live in a surrogacy friendly state, please call or email us.

Additionally, you should think carefully about your availability and willingness to be pregnant and to commit at least a year of your life to helping your intended parents achieve their goal of having a baby.  Although the surrogacy process is greatly rewarding, it can also be an intense, challenging and emotional journey.

Our Surrogate Mothers Are Special and We Treat Them as Such!

We know that without women willing to carry a baby for another individual or couple there would be no surrogacy option for our Intended Parents. Expect to be treated with respect and gratitude by our staff and the Intended Parents that you meet. We will make sure that you understand the surrogacy process to the fullest and that you have the tools to make the best decisions for both you and your family. We take great pride in the fact that each and everyone that you come into contact with has the expertise to guide you through our unique program. We are here for you!

We require that the Intended Parents open an Escrow account for each of our Surrogate Mothers, into which they will place the funds for the surrogacy arrangement before the Embryo Transfer. That way, payments will always be sent on time and you will not have to remind anyone to send them. Even if something were to happen to your Intended Parents, you will still receive the appropriate compensation because the funds have already been set aside for you. In addition, we require that the Intended Parents designate a guardian so you will not have to worry about what will happen to the baby you are carrying.

Getting Started

If you are interesting in becoming a Surrogate Mother, please review the steps below.

  1. Complete the online Surrogate Inquiry Form

  2. If you meet the preliminary qualifications for our program (see below), we will send you an information package and questionnaire via e-mail or U.S. Mail, at your option.  The questionnaire will ask you for detailed information about yourself and your health status. The questions will cover a broad range of subjects so that we, and eventually your Intended Parents, can get to know you better.

  3. We will review your questionnaire, and if it is determined that you meet our program requirements, we will arrange for a consultation and interview either in person or over the phone.  During the consultation, we will educate you about assisted reproduction and surrogacy and answer any questions you may have.

  4. If you qualify for our program and you would like to register as a Surrogate Mother, we will begin the pre-screening process, which includes you and your spouse/partner meeting with a psychologist.

  5. Once you are registered with us, we will work diligently and quickly to promote your profile and match you to an individual or couple as soon as possible.

Preliminary Qualifications

The first step before applying and begin the screening phase of our program is to see if you meet the preliminary requirement listed below.

  • Must be a resident of a surrogacy friendly state

  • Between the ages of 21 and 40

  • Uncomplicated Pregnancy/Birth at least once (C-section OK)

  • Maintains a healthy lifestyle (no STDs, HIV, does not or rarely consumes alcohol, some medications OK)

  • Parenting own child/children

  • No criminal history

  • Health Insurance with maternity coverage

  • A stable income

  • Reliable transportation

  • No Medicare/State Insurance

  • A non-smoker/not exposed to smoke

  • No history of drug or alcohol problems

  • Have a spouse, significant other or another support system who is supportive of the decision to become a Surrogate Mother

  • Have a healthy weight/height ratio (BMI)

  • Enjoys being pregnant and all it involves

  • Has the desire to change a life

Matching Process

You will be matched with Intended Parents based on your preferences, personalities, clinic requirements, and other requirements that are important to you. We want you to feel comfortable with your Intended Parents. After all, you all will traveling this path together for many months and being comfortable with each other is critical to a successful journey.

Our matching process is quite simple.

  1. Once you are registered as a Surrogate Mother with us, your profile (excluding identifying and confidential information) will be presented to couples who most closely match your preferences and requirements.

  2. When a couple indicates that they are interested in learning more about you, we will send you a copy of their profile for review.

  3. If you and the couple express interest in working together, we then set up and facilitate a 3-way phone conference. If the phone conference is successful, then the next step is usually arranging a face-to-face meeting. One of our experienced staff members will be on hand to facilitate this meeting as well.

  4. If both parties decide to work together, you will be matched, and we will assist in setting up appointments for the medical and psychological testing. We will also make sure that each party has legal representation and that contract obligations are discussed.

Once you agree upon a fee and the specifics of your surrogacy arrangement, your Intended Parents' attorney will draft a fair and reasonable agreement and send a copy to your attorney.  You and your attorney will then review the agreement, suggest any changes (additions, deletions or alterations) and review the final version before you sign it.

Any attorney fees are paid by the intended parents.

Ongoing Support

Our personalized support program for our Surrogate Mothers ensures you will have a wonderful surrogacy experience.  As a Surrogate Mother with Kinsey Law Group, P.C., you determine as much or as little contact that you want with us (weekly vs monthly check-up calls).  You will will have access to one of our Surrogate Mother Coordinators throughout the process, from the time you are accepted into the program until your post-partum checkup. We will always be there to answer any questions or concerns that arise day or night.

Compensation

You will determine your fee for carrying and delivery.  Here are some guidelines to help you determine an appropriate fee.

First time Surrogate Mother: $18,000 to $20,000 (paid in installments)

Experienced Surrogate Mother: $25,000 to $35,000 (paid in installments).

Additional compensation is provided for the following:

  • Monthly expense allowance (including local mileage, child care and lost wages while attending appointments to carry out the terms of the agreement, over the counter medications, telephone calls, other incidental reasonable expenses)
  • Maternity clothing
  • Embryo transfer
  • Carrying multiples
  • Invasive procedures (amniocentesis, Chorionic villus sampling or CVS, termination, selective reduction, D&C)
  • C-section (if medically required)

If travel is required, all hotel, transportation, meals, chid care and lost wages will be provided.

Your intended parents are responsible for all expenses such as travel, medical fees, child care, medications, attorney’s fees, phone calls, etc.  If your monthly allowance does not cover your expenses for a particular month, save your receipts and you will be reimbursed.

Things to Consider

Here are some things you need to consider before becoming a Surrogate Mother.  This list is by no means complete, but it gives you a starting point.

  • Is surrogacy legal in your state?
  • Will this be a traditional or gestational surrogacy?
  • Will you have ongoing support 24 hours a day to answer any questions?
  • Will you have someone to help you navigate the process and resolve any issues that may arise between you and your intended parents?
  • Will your intended parents be screened (a background check and a psychological assessment)?
  • Will you have your own attorney who will look out for your interests?
  • How will you feel about remaining in contact with the intended parents after the birth?
  • How do you feel about selective reduction or abortion?
  • How do you feel about carrying multiples?
  • How do you feel about working with gay couples?
  • If compensation is involved, how much do you feel is appropriate?
  • Do you have a support system in place in the event of doctor ordered bed rest?
  • Are you comfortable with giving yourself (or having someone administer) numerous shots?

Not only will we go over the benefits of becoming a Surrogate Mother with us in detail, but your own personal attorney will review your contract with you and explain anything that is not clear to you. We make sure you know what to expect.
